Jeg ville ikke skrive både "Dear Sir" og "To whom it may concern". Jeg ville nok bruge "Dear Sir or Madam", det andet virker ikke så venligt.
Skriv hellere direkte hvad de "unavoidable circumstances" er---at du bor i udlandet.

Ellers ser det fint ud for mig.

(eller kan jeg godt skrive as I live abroad istedet)
Therefore, please accept this letter as (confirmation that I, XXXX hereby)evidence of my authorizing(e) Mr. XXX to act as my personal representative and (to) submit (the) any required documents on my behalf (of me).
(Along with the letter I am sending) I enclose  the required documents (which includes) including 1)XXXX, 2)’XXX  Please verify these and consider my case with special interest.
